Your Medicare card holds your Medicare number, which is required to claim from Medicare, receive PBS medicines and placing children on the Australian Childhood Immunisation Register.

Medicare cards for over 15 year olds

A young adult over the age of 15 years may be enrolled on their own Medicare card. To transfer a person over the age of 15 years on to a Medicare card of their own they will need to attend a Medicare office with identification and complete a Medicare Copy/Transfer Application Form. It should be noted, that if a parent wishes to do this on behalf of the young adult, it is necessary for the young adult and the parent to complete the Medicare Copy/Transfer Application Form [PDF, 60Kb]PDF reader required.

How do I get a duplicate card?

A duplicate card may be requested by:

  • calling Medicare on 132 011,
  • attending a Medicare Office (a form is not required),
  • post a request to Medicare, GPO Box 9822 in your capital city (a form is not required), or
  • using Medicare Australia's online services.

How do I get a replacement card?

You will note on your Medicare card there is an expiry date on the bottom right hand corner. If this date has recently lapsed, and you are not changing your address or your address has not been changed in the last four weeks, a replacement card may be requested by:

  • calling Medicare on 132 011,
  • attending a Medicare Office (a form is not required),
  • post a request to Medicare, GPO Box 9822 in your capital city (a form is not required), or
  • using Medicare Australia's online services.

If the expiry date has lapsed by more than six months, you will need to attend a Medicare Office with proof of identity (eg. passport or driver's licence) plus proof of residency (eg. rates notice, rental contract, employment contract).

If you are residing or holidaying overseas, your Medicare card cannot be reissued to you until you return to Australia.

How do I change my card details?

Changes of address may be made by calling Medicare on 132 011 or using Medicare Australia's online services but other changes such as change of name, gender or date of birth must be made at a Medicare Office. Documentation that supports the change will be required.

Adding a newborn, copying children from one card to another and transferring person/s from one card to another must be made at a Medicare Office.

How do I change my address?

It is important that you keep your Medicare address up-to-date. If Medicare does not have your current address, you may not receive important information which could cause delays in processing claims for Medicare benefits.

You can change your address by:

  • calling Medicare on 132 011,
  • attending a Medicare Office (a form is not required),
  • post the address change to Medicare, GPO Box 9822 in your capital city (a form is not required), or
  • using Medicare Australia's online services.

How do I change my name?

To change your name you will need to visit a Medicare office with your current Medicare card and appropriate documents to show that you are now known by a different name in the community.

This can be in the form of:

  • current driver’s licence
  • birth certificate
  • marriage certificate
  • deed poll
  • legal document
  • utility accounts

Once the name is changed, the old card is cancelled and is kept by Medicare. At this time you will be advised of your new Medicare card number and your new card will be forwarded to you by mail.

In extreme cases, if you live in a remote area or cannot reasonably visit a Medicare office, you can send a signed request including certified copies of the above documents and your current Medicare card to GPO Box 9822 in your capital city. Until the new Medicare card arrives it is advisable that you record your Medicare card number for Medicare claiming purposes.
