Australian Childhood Immunisation Register

- Your child’s immunisation history can help you brochure

- Immunisation History form
– used to record a child's vaccination details that may be missing from the Immunisation Register. This form must be completed by a doctor or immunisation provider and sent to the Immunisation Register.
Note: printed copies of this form will not appear in triplicate.
- Medical Contraindication form
– used when a child cannot be immunised because of a medical condition. This form must be completed by a doctor or an immunisation provider and sent to the Immunisation Register. - Conscientious Objection form
– used to record a parent's personal, philosophical, religious or medical belief that immunisation should not occur. This form must be signed by a doctor or immunisation provider and sent to the Immunisation Register.

- Medicare levy exemption certification form

If you were a resident of a Reciprocal Health Care Agreement country prior to your arrival in Australia, have applied for permanent resident status (conditions apply), are a New Zealand citizen who is residing in Australia or a permanent resident visa holder you may be eligible for enrolment in the Medicare program, therefore ineligible to claim exemption from payment of the Medicare levy. - Medicare notification of deceased person form
