Oral and Maxillofacial Surgeons

From 1 November 2004 dental practitioners must have medical qualifications and be recognised as a specialist by Medicare Australia before being able to access Medicare benefits for cleft lip and cleft palate services.

For more information regarding specialist recognition as an Oral and Maxillofacial surgeon please call the provider eligibility section on 02 6124 7149.

The certification of cleft condition form is used to register a prescribed dental patient. For more information regarding the cleft lip and cleft palate scheme please call 1300 652 492.
