You can lodge these forms by post to GPO Box 9822, in your Capital City or by fax to the numbers listed on the form. If forms are lodged by fax, please note the retention requirements required for auditing purposes. These requirements are listed on the form. Medicare Allied Health and Dental Care Initiative
The new Medicare allied health and dental care initiative allows chronically ill people who are being managed by their GP under an Enhanced Primary Care (EPC) plan access to Medicare rebates for allied health services.
Referral forms are available on the Department of Health and Ageing
website or by faxing a request to (02) 6289 7120.
