Medicare Australia is committed to continually providing increased access and delivery of appropriate services to our customers through online service delivery. As part of this, a number of forms and publications are available to download or to view online.
Certain forms are also now available to be lodged electronically. You are welcome to remain with the traditional methods of lodgment, such as completing hard copy forms and posting them, or alternatively, you can now choose to access information from this website and send it in electronically.
Please note, to confirm which lodgment method is acceptable, you can refer to the specific form under each individual health program. Each form will specify which lodgment method is available and lead you step-by-step on how to lodge your form.
It is also essential to know that if you choose to use the electronic lodgment method, you must conform with Medicare Australia stipulated conditions to ensure that the form is received and accepted. These conditions are included in the instructions section on how to complete your form.
Foreign language support on Medicare Australia's programs and other publications are also available.
The Schedule of Pharmaceutical Benefits lists the medicines available under the Pharmaceutical Benefits Scheme. Copies of the Schedule may be obtained by completing the Schedule of Pharmaceutical Benefits order form [PDF, 28Kb]

Electronic Referral and Requests
With the introduction of the Electronic Transactions Act 1999 and as part of the Government's Online Strategy, Medicare Australia is now able to provide the IT standards that Medicare Australia requires for electronic transmission, scanning and storage of Referrals to specialists/consultant physicians and Requests for pathology and diagnostic imaging services. If you would like to transmit, scan and/or store Referrals or Requests electronically, please refer to the following Medicare Australia IT standards.
