IVF/GIFT program

General information

You can now order IVF/GIFT medicines online through Health Professional Online Services (HPOS).

The MBS item codes and descriptions for IVF/GIFT treatments covered by the Pharmaceutical Benefits Scheme (PBS) are explained in the following table.

13202 Assisted reproductive technologies superovulated treatment cycle that is cancelled before oocyte retrieval, involving the use of drugs to induce superovulation and including quantitative estimation of hormones and ultrasound examinations, but excluding artificial insemination, transfer of frozen embryos or donated embryos or ova or a service to which item 13200, 13201, 13203, 13206 or 13218 applies, being services rendered during 1 treatment cycle
13203 Ovulation monitoring services for artificial insemination, including quantitative estimation of hormones and ultrasound examinations, being services rendered during 1 treatment cycle but excluding a service to which item 13200, 13201, 13202, 13206, 13212, 13215 or 13218

Lodgement

IVF/GIFT clinics approved by the Department of Human Services (Human Services) can submit the IVF/GIFT summary of order form online through HPOS.

You can also send the completed IVF/GIFT summary of order form, Corrifollitropin alfa application form and patient forms to:

Forms

The IVF/GIFT summary of order can be completed and lodged the following ways:

Note: You don’t need to send the patient forms to Human Services if you have submitted the IVF/GIFT summary of order form online. For audit purposes, you need to complete the patient form and keep it for at least two years from the date of supply.

The Corrifollitropin alfa application form must be completed for patients being treated with corrifollitropin alfa. Details of up to three patients can be provided on one form.
