The MBS item codes and descriptions for IVF/GIFT treatments covered by the Pharmaceutical Benefits Scheme (PBS) are explained in the following table.

IVF/GIFT clinics approved by Medicare Australia can forward the completed 'IVF/GIFT summary of order' form, completed corrifollitropin alfa application forms and patient forms to:

Lodge the completed IVF/GIFT summary of order form when submitting patient orders under the IVF/GIFT arrangements.
To order the patient form 'IVF/GIFT arrangements' (form 4049 - the patient form) or for more information on the IVF/GIFT arrangements call 1800 700 270* and select option 1, between Monday to Friday 8.00am to 5.00pm EST.
This form must be completed for patients who are being treated with corrifollitropin alfa. Details of up to three patients may be provided on the same form.
