Applying for certificates

If you have not previously registered with Medicare Australia

To register as a Registered Medicare Australia Communities of Interest (CoI) Site or a Registered Medicare Australia Healthcare Professional Individual you must complete an application form and submit Evidence of Identity documents.

Healthcare Professional Individuals

If you are a Healthcare Professional Individual and you have not previously registered with Medicare Australia, please read the certificate pre-application checklist.

Once you have read the pre-application checklist please complete the Application form for a non-known Medicare Australia Healthcare Professional Individual Certificate.

Please ensure you provide the Evidence of Identity documents with your certificate application form.

Sites

If you are a site and you have not previously registered with Medicare Australia, please read the certificate pre-application checklist.

Once you have read the checklist please complete the Application form for a non-known Medicare Australia Communities of Interest (CoI) Site Certificate.

Please ensure you provide the Evidence of Identity documents with your certificate application form.

Registered Medicare Australia Providers and Registered Medicare Australia Sites that have previously registered with Medicare Australia

For Site Certificates please complete the Registered Medicare Australia Communities of Interest (CoI) Site Certificate application form.

As you have previously registered with Medicare Australia you are not required to submit Evidence of Identity documents.

For Healthcare Professional Individual Certificates please complete the Registered Medicare Australia Healthcare Professional Individual Certificate application form.

As you have previously registered with Medicare Australia you are not required to submit Evidence of Identity documents.

Renewal or Revocation of Certificates

If you wish to renew or revoke your current certificate please complete the request form [PDF, 231Kb]PDF reader required and submit to us. If your details have changed since your initial registration with us, please contact the eBusiness Service Centre for assistance.

Installing certificates for use through Health Professional Online Services (HPOS)

The following installation guides have been developed to help you set up a certificate to access HPOS.

Important:

Individual certificates will provide access to the full range of HPOS information and services.

Location certificates will provide access to the Patient Verification facility only.  When utilising location certificates to access HPOS each computer must have the location certificate installed
