How to submit a subsequent claim

 

When you have successfully logged on to the Midwife homepage and selected a midwife record the following screen will display. Click on the '+ Add Subsequent Application' hyperlink next to a completed claim.

Step 1

 

The following Subsequent Application Claim screen will display with a number of tab panes all populated with data from the original claim. Enter all required data in each tab pane for the subsequent claim, select the 'Next' button to continue.

Step 2

 

When all tab panes have been completed, select the 'Next' button to continue.

Step 3

 

The Claim Summary screen will display. After checking all entered data is correct, press the 'Submit' button.

Step 4

 

The Claim History screen will display, showing the submitted claim in the table.
