How to submit a new claim

 

When you have successfully logged on to the Midwife homepage and selected a midwife record the following screen will display. Click on the 'Create New Claim' hyperlink.

Step 1

 

The following New Claim screen will display with a number of tab panes. Enter all required data in each tab pane, and select the 'Next' button to continue.

Step 2

 

When all tab panes have been completed, select the 'Next' button to continue.

Step 3

 

The Claim Summary screen will display. After checking all entered data is correct, press the 'Submit' button.

Step 4

 

The Claim History screen will display, showing the submitted claim in the table.
