Creating a Medical Certificate(SU683.1203)

 

Go to the Centrelink Forms page by selecting Centrelink forms in the HPOS Main menu.

Step 1

 

The Centrelink Forms page will display. Select Medical Certificate(SU683.1203). The Medical Certificate instructions screen will be displayed.

Step 2

Declaration - in order to continue you must agree to the declaration. If you agree, select the declaration box and select the Continue button.

The bar at the top of the screen shows where you up to in the form. At any stage whilst completing the form you can:

Cancel - close the form at the point that you have reached. If some pages have been saved, the form will be saved as a draft that you can return to at any stage.

Continue - move to the next page. Mandatory fields are required to be completed before moving to the next page.

Read the instructions carefully and select Continue.

 

On this page you are required to enter the Patient Details.

Note: All fields are mandatory.

Step 3

Cancel - close the form at the point that you have reached. If some pages have been saved, the form will be saved as a draft that you can return to at any stage.

Save Only - save the page that you are on. Mandatory fields are not required to be completed to use the save only function.

Save and continue - save the page you are on and move to the next page. Mandatory fields are required to be completed before moving to the next page.

 

Enter the patient's primary Medical Condition(s).

Note: All fields are mandatory.

Step 4

Once you have completed all mandatory fields select one of the following options.

Cancel - close the form at the point that you have reached. If some pages have been saved, the form will be saved as a draft that you can return to at any stage.

Save Only - save the page that you are on. Mandatory fields are not required to be completed to use the save only function.

Save and continue - save the page you are on and move to the next page. Mandatory fields are required to be completed before moving to the next page.

 

On this page you are required to complete the Capacity to work or Study details, all fields are mandatory and you will not be able to progress without this information.

Step 5

Once you have completed all mandatory fields select one of the following options.

Cancel - close the form at the point that you have reached. If some pages have been saved, the form will be saved as a draft that you can return to at any stage.

Save Only - save the page that you are on. Mandatory fields are not required to be completed to use the save only function.

Save and continue - save the page you are on and move to the next page. Mandatory fields are required to be completed before moving to the next page.

 

The Certification by Medical Practitioner screen displays the Confidentiality of Information and also displays the authenticated user provider details.

Updates to personal details will not be saved through this form. To permanently update your provider details you must go to the 'Provider personal details' page in HPOS.

Step 6

Cancel - close the form at the point that you have reached. If some pages have been saved, the form will be saved as a draft that you can return to at any stage.

Save Only - save the page that you are on. Mandatory fields are not required to be completed to use the save only function.

Save and continue - save the page you are on and move to the next page. Mandatory fields are required to be completed before moving to the next page.

 

The form is now complete. Check the information in the Review and Submit screen. If any details are incorrect, you can use the 'Update' link under each heading of the form to go back and re-enter the details.

If all details are correct, select Submit and the form will be submitted to Centrelink.
