If you suspect someone of committing fraud, please complete as much of this form as you can. The more information you provide, the better but do not worry if you are unable to answer some of the questions.
You are under no obligation to provide your personal details when giving us information. However, if you give your name and a contact number, it allows us to ask you for more information later if necessary.
If you choose to remain anonymous, we would appreciate you giving as much information as you can, as we will not be able to contact you again for more details.
